An abscess forms when infection causes pus to collect within a confined space in the tissue, and as it enlarges, it can cause pain, swelling and inflammation. In colorectal practice, abscess drainage is commonly performed for infections affecting the area around the anus and rectum, although abscesses can occur in other parts of the body as well.

Before the procedure

The procedure is usually performed under general anaesthesia. Drainage under local anaesthesia can be performed in selected cases. Local anaesthesia will numb the area, ensuring that the procedure can be performed with minimal discomfort.

During the procedure

Once the area is numbed, a small incision is made over the abscess to allow the trapped pus to drain. The infected cavity may then be gently cleaned with sterile fluid to remove remaining debris or bacteria.

Depending on the size of the abscess, the cavity may be left open or a small dressing may be placed inside to allow continued drainage while healing takes place.

Why can't I just drain the abscess myself at home?

While the idea of popping or cutting it open yourself might seem like a faster fix, this is not something to attempt without medical supervision, and doing so can lead to serious complications. You should not cut the abscess yourself because:

  • You can't see how deep or how connected it is What looks like a small bump on the surface can have tracts running deeper into surrounding tissue. Cutting blindly risks missing pockets of pus entirely or, worse, pushing infection further in.
  • Risk of spreading the infection — Squeezing or cutting at home can force bacteria into the bloodstream (bacteremia) or push it into deeper tissue planes, which is how localised abscesses turn into much more serious things like fasciitis or sepsis.
  • No sterile field — Home tools and environments aren't sterile, so you're introducing new bacteria into an already-infected, open wound.
  • Anorectal anatomy specifically is unforgiving — Near the anus, abscesses often track along fascial planes and can involve the sphincter muscles. A clean surgical drainage avoids damaging those muscles; a DIY cut easily can.
  • Incomplete drainage = recurrence or fistula — If the cavity isn't fully opened and packed/drained properly, it can wall off again, or develop into a chronic fistula that needs more complex surgery later.
  • No pain control or proper aftercare — Without anesthesia, people often stop short of full drainage because of pain, leaving infection behind.
